Patients fulfilling pre-defined clinical criteria and programme requirements and who have been attending HA Outpatient Clinic for at least 12 months will be invited to join the Programme to receive primary care service from the private sector.

Issue 28 - Patient Empowerment Programme

Dear Doctors,

We are pleased to inform you that you may refer your GOPC PPP patients to the Patient Empowerment Programme (PEP) starting from 12 July 2019.

PEP is one of the PPP programmes jointly organised by the Hospital Authority and selected non-governmental organisations (NGOs). Patients who are referred to join PEP will attend the empowerment sessions in the community to acquire disease-specific knowledge, self-efficacy enhancement and lifestyle modification information, which will further enhance their knowledge and ability in self-management of chronic diseases so as to prevent possible complications.

You may refer to the PEP website www.ha.org.hk/ppp/pep and the Appendix for details. Some PEP pamphlet (as attached) will also be sent to your clinic to facilitate communication with patients.
